Positions can be combined to make one 36hr position.
Working in a community residential program for 4 men with acquired brain injuries as an important part of their residential services team.
Provide supports to participants with disabilities in residential settings including wound care, medication administration, g-tube care, health care coordination, and advocacy Assists with the development and implementation of health care standards, policies and procedures.
Provides training and consultation to direct care staff regarding policies and procedures according to Medication Administration Program (MAP) policy.
Reviews all incident reports involving health and safety issues, including mental health and behavioral incidents.
Oversees MAP Program at the residence by conducting routine audits of documentation system.
Conducts reviews of health care documentation to ensure program compliance with the provider's recommendations as well as MAB, DDS policies and the MAP policies.
Identifies and facilitates the utilization of appropriate community resources.
Transports participants to medical appointments and provides advocacy at the appointment.
Administers medications and treatments as prescribed.
Works as a part of the wider LPN/RN nursing team.
CERTIFICATES, LICENSES, REGISTRATIONS Current Licensed Practical Nurse (LPN) License Current, valid, US drivers' license All candidates must be able to successfully pass a CORI, reference, multi-state background and driving record check.
